This hard-hitting ballad was released as the second single from the rock band Breaking Benjamin's fourth studio album, Dear Agony. The song adopted the extended title, "Give Me a Sign (Forever and Ever)" for its single release.

The song's hospital-set music video was directed by Nigel Dick and features artwork from Breaking Benjamin's four albums. A picture of the cover art from Saturate is hanging up in the child's room, the old man has the Breaking Benjamin tattoo on his finger from the cover of We Are Not Alone, a visitor whose face can't be seen is the cover from Phobia and the woman's room features the MRI scan of singer Ben Burnley's head from the cover of Dear Agony. Prior to the video's debut, Burnley had commented that he hoped fans would "recognize and be excited by the inclusion of certain key characters from the band's past". The woman in the hospital bed is Jane from the clip for "The Diary of Jane" and the faceless visitor is the "Evil Angel" from the song of the same name.
