This track about redemption and forgiveness is a deeply personal song to vocalist James Hetfield. He told The Sun August 2, 2008: "It takes me to another level. If there was any song that I could listen to 100 times in a row that would be it."
Hetfield told The Guardian September 5, 2008 that the song's concerns how you "can't forgive anyone until you've forgiven yourself."
This is the third of Metallica's Unforgivens. The first two start with the same guitar riff, but "The Unforgiven III" starts with a piano riff.
