In this song, Creed frontman Scott Stapp sings about those who oppress the band with their words and hatred. He seems to be referring to the media and critics who lashed out at Creed, and asks what they would do if faced with a similar situation.
"What If" was released on Creed's second album, Human Clay, in 1999. This was two years after their debut, My Own Prison, but that album took a while to catch on, so it didn't earn airplay until 1998, and songs like "What's This Life For" and "One" were still getting played well into 1999, so when Human Clay came out, there was no break from Creed on the airwaves. "Higher" was the first single, followed by "What If." Next came "With Arms Wide Open," which was a #1 hit but burned out many listeners on the band. After their third album, Weathered, was released, radio saturation made listeners even more weary of their sound. The musicians in the band were also getting sick of Scott Stapp, who was dealing with addiction and mental health issues. They split up in 2004, with the rest of the band forming Alter Bridge. Creed returned in 2009 for one more album and got together again in 2024 for more touring.
"What If" was used in the 2000 movie Scream 3 - Creed helped produce the soundtrack. The music video shows the band getting offed one-by-one by the infamous Ghostface from the franchise. The video features David Arquette in his Scream role.
